What is a Follicle Tracking Scan?

Follicle tracking is a series of transvaginal or transabdominal ultrasound scans performed at specific points in your menstrual cycle to monitor the development of ovarian follicles. Each follicle contains a developing egg; as the dominant follicle grows (typically to 18–25mm), it indicates that ovulation is imminent. The scan also assesses the endometrial lining thickness and pattern, which is important for embryo implantation. Most women require 2–4 scans per cycle.

What This Scan Can Detect

Dominant Follicle Growth
Serial measurement of the dominant follicle diameter to track its development from baseline to pre-ovulatory size (18–25mm), indicating imminent ovulation.
Ovulation Confirmation
Identification of follicle rupture (ovulation has occurred) by the disappearance of the dominant follicle and the appearance of free fluid in the pelvis.
Endometrial Thickness & Pattern
Assessment of the uterine lining thickness and trilaminar pattern, which should be at least 7–8mm with a triple-line appearance for optimal implantation.
Antral Follicle Count
Counting of small resting follicles in both ovaries, which provides an indication of ovarian reserve and response to stimulation.
Ovarian Cysts or PCOS Features
Identification of ovarian cysts or polycystic ovarian morphology that may affect follicle development and ovulation timing.

What Happens During Your Scan

Baseline scan (Day 2–3)

The first scan is performed early in your cycle to establish a baseline antral follicle count and check for any cysts before treatment or monitoring begins.

Mid-cycle monitoring (Day 9–11)

A second scan assesses follicle development. If a dominant follicle is identified, the timing of subsequent scans is planned accordingly.

Pre-ovulatory scan (Day 12–14)

When the dominant follicle reaches 16–18mm, a further scan is arranged to confirm readiness for ovulation and assess the endometrial lining.

Ovulation trigger guidance

If you are using an hCG trigger injection, your sonographer will advise on the optimal timing based on follicle size and endometrial appearance.

Post-ovulation confirmation

An optional final scan confirms that ovulation has occurred, providing reassurance that the cycle has been successful.
